exclusively consultative in nature and do not include, involve, or constitute
the provision of care or treatment to any patient or individual.
WoundLink Consulting Services are provided through WoundLink representatives
directly to health care professionals, and not to patients.
In connection with such Services, certain forms and notes may be provided to
health care professional.
These forms and notes are being provided as a courtesy only and are intended for
the exclusive use by the health care professional's facility's quality
assurance committee. Health Care professional understands and agrees that these
forms and notes, and any of their derivatives, are intended exclusively for
quality assurance purposes, are to be kept separate and apart from any
residents medical record and are not to be included or incorporated into the
residents medical record in any form
It is understood in the provision of these consulting services that WoundLink
cannot and will not touch patients or provide direct care to patients, (2)
cannot and will not take orders for patient care, (3) cannot and will not
alter, supplement, augment or modify patient medical records in any form, and
is not responsible for the diagnosis, treatment or follow-up care provided to
patients.
Health Care Professional retains full and complete professional and
administrative responsibility for providing services that meet professional
standards and practices that apply to the provision of such services, as well
as for the timeliness of such services.
